Rates:
Initial 15-minute phone consultation session = FREE
Initial assessment session (75 minutes) = $275
Regular 30-minute session = $120
Regular 45-minute session = $200
Regular 60-minute session = $240
10-session packages (provides 5% discount, payment made up front and nonrefundable):
30-minute session package = $1140
45-minute session package = $1900
60-minute session package = $2280
Dr. Kaufman is available to observe live performances. Rates for this service can vary depending on travel and the length of the event.
Payment and Insurance:
Payment is expected at the time of service. Cash, checks, and credit or debit cards are accepted. Dr. Kaufman does not participate in any insurance networks, but he does provide receipts which are submittable for possible out-of-network reimbursement according to the terms of your insurance policy. Submitting receipts is usually a straightforward process and Dr. Kaufman can help guide you through it.
Dr. Kaufman is recognized as an authorized (certified) Tricare provider, but is not considered in-network by this or any other insurance company.
No Surprises Act Notice:
Dr. Kaufman is an out-of-network provider, meaning he has not signed a contract with your health insurance plan to provide services. Out-of-network costs can be higher than in-network costs for the same service and might not count toward your plan’s deductible or annual out-of-pocket limit. You are not required to get out-of-network care and can always choose a provider in your plan’s network. You also have the right to a Good Faith Estimate for the total expected cost of out-of-network services. For additional information regarding the No Surprises Act and your rights, you can visit cms.gov.
Cancellations:
Please notify Dr. Kaufman of a need to cancel a scheduled session at least 2 days in advance. Otherwise, you may be required to pay a fee for the session.
